Provides supports and services to meet the needs of those living with or at risk for HIV/AIDS and other chronic medical conditions and their loved ones, regardless of orientation, lifestyle or belief. This is accomplished by service and programs including: health home care coordination, intensive and supportive case management; Project Safe Point (syringe disposal, access/exchange services, recovery readiness counseling, treatment access, HIV/Hepatitis testing and opioid overdose prevention); linguistic services (medical interpretation for those with limited English proficiency); housing support services; personal care closet; and emergency financial assistance. The agency functions in a consultative manner for the entire Catholic Charities system and the Diocese in matters related to HIV/AIDS.

Services include, but are not limited to the following: residential placement; residential and respite services offered to individuals in their own homes and community settings; family support services; independent service coordination; Traumatic Brain Injury (TBI) services; advocacy; referral; Prader-Willi Syndrome services.

CMS provides family-oriented and community-based programs, which enhance the dignity, quality and sanctity of human life through individualized social services and community education programs. Offers a continuum of care for pregnant adolescents and young parents including case management, goal-directed counseling, childbirth education, parent education, support and advocacy. Group residence and family care for children. Prevention/supports for families at risk. Adoption, foster care, day care and respite services.
